Health at Home

Health at Home, LLC is a new Medicare-certified home healthcare agency affiliated with locally owned Graceworks at Home and Graceworks Lutheran Services.

Health at Home, LLC recently achieved accreditation by The Joint Commission. Health at Home provides skilled nursing care, therapy, medical social services and home health aide assistance for homebound patients. Services include nursing care, rehabilitative therapy, medical social services and home health aide assistance.

Health at Home staff abide by the same policies, procedures and organizational values that guide all Graceworks staff and services, including Bethany Village and the Bethany Rehabilitation Center. Health at Home allows seniors to recover at home where they are comfortable and cared for by familiar professionals.

Health at Home, LLC services are currently only available to those living in the South Dayton neighborhoods or at Bethany Village.
