State-Tested Nursing Assistants
State-tested nurse assistant (STNA) positions available on all shifts to care for residents in our long term care units and rehabilitation center.  Qualified candidates will have completed the State of Ohio Nurse Aide Training Program and have their 75-hour Nursing Aide Training Certificate. Must have a high school diploma or GED.


MDS Nurse

Full-time (32 hours/week):  Responsible for managing and coordinating the MDS.  Preferred candidates will possess current Ohio professional nurse licensure and have at least three years MDS experience in a long-term care facility.  Must have demonstrated knowledge and skills in completing the assessment process including computer competency.  Strong clinical skills, communication skills and team-building skills desired.

Admissions Registered Nurse (Rehab)
Full-time (35 hours/week; 2nd shift):  Responsibilities include completing or insuring that all admission paperwork and assessments are completed in an accurate and timely manner.  Assesses and plans nursing care for new admissions in Bethany’s Rehab Unit..  Must be a current Registered Nurse from an accredited school of nursing.  Long-term care experience is preferred but not necessary.  An outstanding customer service attitude is required.

Physical Therapist
PRN:  Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, documentation, and modification of all patient/resident programs. Administers physical therapy in compliance with Federal, State, local, and Bethany Village standards, policies, and guidelines.  Qualified candidates will have a Bachelor’s degree from an accredited college in Physical Therapy (Master’s degree preferred) and a current Ohio Physical Therapy license.  At least two years physical therapy experience in long-term health care setting is required.

Occupational Therapist

PRN:  Responsible for evaluation, plan of care, treatment, re-evaluation, discharge and appropriate and effective communications to residents.  Qualified candidates will have a Bachelor’s degree from an accredited Occupational Therapy program (Master’s degree preferred), current Ohio license in Occupational Therapy and CPR certification.  At least two years experience, preferably in a long-term health care setting, is required.

Therapy Aide

PPart-time (20 hours/week):  Responsible for transporting patients/residents as scheduled to therapy appointments.  High school diploma/GED and STNA certification required.  Experience in providing patient/resident transportation and care preferred.  Must demonstrate good verbal and written communication skills.            

Dietary Aide
Part-time (20 hours/week):  Position responsible for providing appropriate assistance with activities of daily living for residents of Assisted Living.  Provides secure and safe environment; attempts to meet the psychosocial needs of residents or referral to the proper person.  High school diploma or GED required.  Previous experience in health care preferred.

Certified Occupational Therapy Assistant (C.O.T.A.)
PRN: Under the supervision of an Occupational Therapist, will perform treatments and provide occupational therapy instruction and demonstration for our residents.  Qualified candidates will have current Ohio certification as an Occupational Therapy Assistant.

Driver

PRN:  Provides transportation services to residents.  High school diploma or GED required.  Must have current CDL License (Class C-Passenger with air brake endorsement) and excellent driving record.  Knowledge of Dayton area and at least one year of experience driving vehicle which carries at least 10 passengers preferred.

Cleaner

FFull-time: Position primarily responsible for shampooing, spot cleaning and vacuuming rugs, sweeping, mopping and refinishing hard surface floors, trash removal, cleaning common areas and other duties as assigned.  Previous experience in housekeeping or janitorial field preferred, but will consider applicants with excellent work and attendance history. High school diploma or GED required.

Laundry Worker
Full-time (40 hours/week): Position provides laundry services including washing, drying, folding, sorting and delivery of clothing/linens. High school diploma or GED required.  Qualified candidates must be able to operate all job-related equipment, carry out both verbal and written instructions, and have excellent communication and customer service skills.  Lifting is required up to 50 lbs. occasionally.

Housekeeper
PPRN:  Positions provide housekeeping and cleaning service in all household and common areas of our facilities.  Qualified candidates must be able to operate all job-related equipment, carry out both verbal and written instructions, and have excellent verbal and customer service skills.  Lifting is required up to 50 lbs. occasionally. High school diploma or GED required.  Previous housekeeping experience in a health care facility preferred.

Dietary Aide
PRN: Position provides quality nutritional care to residents through accurate and sanitary tray assembly and cleaning activities.  Previous experience working in food service (fast food, schools, cafeteria, restaurant or health care) is preferred but not necessary.  Must have high school diploma or GED, or currently enrolled in either.
