Positions available at Bethany Village in Centerville, Ohio
State-Tested Nursing Assistants
State-tested nurse assistant (STNA) positions available on all shifts to care for residents in our long term care units and rehabilitation center. Qualified candidates will have completed the State of Ohio Nurse Aide Training Program and have their 75-hour Nursing Aide Training Certificate. Must have a high school diploma or GED.
Registered Nurse
Full-time (7am-7:30pm): Responsible for providing superb patient care for residents in our rehabilitation center. Qualified candidates will possess current Ohio registered nurse licensure, strong assessment skills, great customer service skills and previous health care experience.
Licensed Practical Nurse
Part-time (8 hours/week) and PRN: Responsible for providing superb patient care for residents in our long term care units and rehabilitation center. Qualified candidates will possess current Ohio practical nurse licensure, strong assessment skills, great customer service skills and previous health care experience.
Daily Living Assistant
Full-time (7am-3:30pm) and part-time (3:00pm-11:30pm and 11pm-7:30am): Position responsible for providing appropriate assistance with activities of daily living for residents of Assisted Living. Provides secure and safe environment; attempts to meet the psychosocial needs of residents or referral to the proper person. High school diploma or GED required. Previous experience in health care preferred.
Unit Secretary
Full-time: Position is responsible for maintaining daily census listing, room assignment listing, physician/resident list, and other lists as needed for a 49-bed unit; performing routine audits of in-house charts to ensure completeness and accuracy; filing lab/diagnostic reports and other chart forms upon receipt; auditing MARs/TARs for completeness; maintaining Medicare certification forms on in-house Medicare skilled residents; compiling statistics for the nursing unit as directed; scheduling and coordinating resident appointments and transportation as needed; and placing orders for unit purchases and work orders for housekeeping/maintenance issues as needed. Qualified candidates will possess a high school diploma or GED with some medical records training. Strong Microsoft Office skills needed including Word, Excel and Outlook. Must be highly organized with exceptional communication and written skills. Experience in hospital or long-term care preferred. Ability to work without constant supervision required.
Physical Therapist
PRN: Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, documentation, and modification of all patient/resident programs. Administers physical therapy in compliance with Federal, State, local, and Bethany Village standards, policies, and guidelines. Qualified candidates will have a Bachelor’s degree from an accredited college in Physical Therapy (Master’s degree preferred) and a current Ohio Physical Therapy license. At least two years physical therapy experience in long-term health care setting is required.
Certified Occupational Therapy Assistant (C.O.T.A.)
PRN: Under the supervision of an Occupational Therapist, will perform treatments and provide occupational therapy instruction and demonstration for our residents. Qualified candidates will have current Ohio certification as an Occupational Therapy Assistant.
Diet Aide
Part-time (every other weekend): Position provides quality nutritional care to over 240 residents through accurate and sanitary tray assembly and cleaning activities. Qualified candidates will possess a high school diploma or GED or be currently enrolled in school. Previous experience working in food service (fast food, schools, cafeteria, restaurant or health care) is a plus.
Village Center Receptionist
Part-time (24 hours/week; 2nd shift): Position is responsible for operating the telephone system, greeting residents, visitors and staff, and processing and routing deliveries and US mail. Must be prepared at all times to handle any type of emergency or disaster. High school diploma or GED required. General knowledge of office operations, high level of interpersonal skills and the ability to multi-task in a fast-paced environment necessary. Qualified candidates will possess at least two years experience in an office environment using Microsoft Office Suite products.
Laundry Worker
Full-time: Position provides laundry services including washing, drying, folding, sorting and delivery of clothing/linens. High school diploma or GED required. Qualified candidates must be able to operate all job-related equipment, carry out both verbal and written instructions, and have excellent communication and customer service skills. Lifting is required up to 50 lbs. occasionally.

Positions available at Graceworks at Home, LLC
Occupational Therapist
PRN: Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, and appropriate and effective communications to clients of Health at Home, LLC, an affiliate of Graceworks at Home. Administers occupational therapy in compliance with Federal, State, local, and Health at Home, LLC standards, policies, procedures, and guidelines. Qualified candidates will have a Bachelor’s degree from an accredited Occupational Therapy program (Master’s degree preferred), current Ohio License in Occupational Therapy and CPR certification. Valid driver’s license and a safe driving record required. At least two years experience in a home health care setting preferred.
Health at Home Coder
PRN: Responsible for performing quantitative and qualitative analysis of the clinical record, ICD-9-CM coding and HCS-D coding rules, correspondence and release of information, HIPAA compliance, as well as the every day clerical functions of the department. Supports Home care’s financial goals through timely processing of records. High school diploma or GED and HCS-D certification or related medical coding certification such as Certified Coding Associate (CCA), Certified Professional Coder (CPC) or Certified Coding Specialist (CCS) required. HCS-D must be obtained within 1 year of hire. Must have strong background in medical terminology and training; working knowledge of ICD-9-CM coding needed. Strong skills in the use of technology, especially Microsoft Windows applications required. Must have excellent organization skills and the ability to be highly productive in a self-directed environment.
Personal Care Aide
Part-time and PRN – flexible hours: Position provides private personal care and light housekeeping services to clients when necessary to maintain their optimum level of independence. Also provides private pay transportation services to residents of Bethany Village. High school diploma or GED and valid Ohio driver’s license with safe driving record required. Must obtain and keep current State-approved first aid certification prior to start of client care. Experience in health care preferred. Must be computer literate.

Program Manager (Hamilton, Ohio)
Full-time: Position responsible for providing a supportive, home-like environment for adults with developmental disabilities in a group home setting. Manages the home and ensures programming is in adherence to all Federal, State and local guidelines. Develops, monitors, and coordinates IPs to allow individuals we serve to develop to their fullest potential. Responsible for staffing and supervision of Direct Care staff. Qualified candidates will have a Bachelor’s degree in Special Education or a related field, a valid driver’s license with a good driving record and at least two years of DD residential experience with two to three years supervisory experience.
Direct Care
Full-time and part-time positions to provide direct care of individuals with developmental disabilities in our group homes. Requires high school diploma or GED and valid driver’s license. Experience with developmental disabilities a plus. Full-time and part-time positions available in Dayton, Middletown, Hamilton and Cincinnati.
