Positions available at Bethany Village in Centerville, Ohio

Director of Nursing
Full-time:  The Director of Nursing oversees the operations of Bethany Village’s Wellness Clinic, Assisted Living, Memory Support, Rehabilitation and Skilled Care units.  Reporting to the Village Administrator, the DON is responsible for providing a superior level of leadership, dedication to the faithful caring of residents/families at our campus, and an excellent understanding of nursing care delivery systems, including state and federal regulations.  Ideal candidates will possess a Bachelor’s degree in nursing, health services or gerontology, a current State of Ohio RN license and at least five years experience in progressive administrative positions requiring management and supervisory expertise.

Registered Nurse/Licensed Practical Nurse
Licensed Practical Nurse:  Responsible for providing superb patient care for residents in our Long Term Care units, Memory Support unit and Rehabilitation Center.  IV certification is required with strong work history including long term care.  Qualified candidates will have strong assessment skills, great customer service skills and previous health care experience.

      Available shift for RN:
          FT 7am-3:30pm, (40 hrs/wk, every other weekend)      

      Available shift for LPN:
          PT 3pm-11:30pm, (16 hrs/wk, every other weekend)

State-Tested Nursing Assistants
State-tested nurse assistant (STNA) positions available on all shifts to care for residents on our Long Term Care units, Rehabilitation Center and Memory Support unit.  Qualified candidates will have completed the State of Ohio Nurse Aide Training Program and have their 75-hour Nursing Aide Training Certificate. Must have a high school diploma or GED.

       Available shifts for RCT (STNA):  
          PT 7am-3:30pm, (16 hrs/wk, every other weekend)
          PT 3pm-11:30pm, (16 hrs/wk, every other weekend)
          PT 11pm-7:30am, (16 hrs/wk, every other weekend)
          FT 7am-3:30pm, (40 hrs/wk, every other weekend)
          FT 3pm-11:30pm, (40 hrs/wk, every other weekend)
          FT 11pm-7:30am, (40 hrs/wk, every other weekend)
                                         
Certified Occupational Therapy Assistant (C.O.T.A.)
PRN: Under the supervision of an Occupational Therapist, will perform treatments and provide occupational therapy instruction and demonstration for our residents.  Qualified candidates will have current Ohio certification as an Occupational Therapy Assistant.

Licensed Physical Therapy Assistant (L.P.T.A.)
PRN:  Under the supervision of a Physical Therapist, performs treatments and provides physical therapy instruction and demonstration for our residents. Qualified candidates will have current Ohio certification as a Licensed Physical Therapy Assistant.

Companion
Part-time: 11pm-7:30am, 16 hours per week with every other weekend.  Provide appropriate assistance with activities of daily living. Observe and report changes in residents’ mental status, physical condition or behavior to the Pavilion Nurse or Manager. Practice universal precautions in all areas of work and perform all duties using proper body mechanics. Assist in escorting residents to dining area, getting food from the salad bar for residents who are unable to select for themselves, and assist in completing and collecting menus.  Restock supplies and food for snacks and monitor temperatures in refrigerators on a daily basis.

Cleaner
Full-time: Position primarily responsible for shampooing, spot cleaning and vacuuming rugs, sweeping, mopping and refinishing hard surface floors, trash removal, cleaning common areas and other duties as assigned.  Previous experience in housekeeping or janitorial field preferred, but will consider applicants with excellent work and attendance history. High school diploma or GED required.

Laundry Worker
Full-time (40 hours per week) and part-time (16 hours per week), includes every other weekend:  Positions available to provide laundry services including washing, drying, folding, sorting and delivery of clothing/linens. Qualified candidates must be able to operate all job-related equipment, carry out both verbal and written instructions, and have excellent communication and customer service skills.  Lifting is required up to 50 lbs. occasionally.

Positions available at Graceworks Enhanced Living locations

Home Supervisor (Cincinnati, Ohio)
Full-time:  To support the program manager in the home to adhere to organization mission, Federal, State and local guidelines.  Implements staffing to ensure equitable coverage based on staff competence, staff experience and client acuity within budget parameters.  Provides 24 hour coverage for absent staff through a pager.  Completes daily operational activities to ensure home runs efficiently.  Qualified candidates must have a Bachelor’s degree from an accredited program in Special Education or related field, medication passing, CPR and first aid certification, valid driver’s license and good driving record.  At least two years MR/DD residential experience.  Previous supervisory experience preferred.

Direct Care
Full-time and part-time positions to provide direct care of individuals with mental retardation/developmental disabilities in our group homes. Requires high school diploma or GED and valid driver’s license. Experience with mental retardation/developmental disabilities a plus. Full-time and part-time positions available in Dayton, Middletown, Hamilton and Cincinnati.
